For those who have appointments, please look over the questions below. If any of your answers are "YES", please call our office at least 24 hours prior to your appointment to reschedule.
1) Within the last month, have you or anyone in your household had flu-like
symptoms such as cough, fever or shortness of breath?
2) Within the last month, have you or anyone in your household been in close
contact with anyone confirmed or being evaluated for COVID-19?
3) Within the last month, have you or has anyone in your household been sick or in
close proximity to someone sick and/or running a fever in the last 48 hours?
Also, due to the distancing regulations, infection guidelines, and trying to maintain the highest level of safety for everyone involved, we are requesting parents to remain in their cars and call our office when you arrive. Our staff will come to get you when it is time for your visit. In addition, please consider limiting the number of people (i.e. family and/or friends) that will be accompanying the patient to the office on the day of appointment so as to comply with social distancing recommendations. We are currently asking for only the patient to come to their appointment, unless medically necessary for others to accompany them.
