Medical records, referrals & fax
Records request: complete the 2020 Medical Release (PDF) and return it to the office or fax it to (941) 391-6937. Requests normally take 7–10 business days.

Cancellations
If you are unable to keep your office visit appointment, we ask for 24 hours notice. If you need to reschedule your procedure or surgery, we ask for 48 hours notice. Cancellation charges may apply.
